2012 Registration Form - .DOC (76 KB)2012 Membership Application Form - .DOC (68 KB)NPZA Scholarship applicants. To apply for a scholarship please complete and return two (2) forms. The first form is the NE APA/ NPZA scholarship application form. ( .DOC). The second form is the United States Citizenship Attestation Form, available on the State of Nebraska Department of Administrative Services website at www.das.state.ne.us.
This form is required because NE APA is a recipient of federal grant funds and as such the individual scholarship recipient receives public benefits and is contracted to comply with Neb. Rev. Stat. §§4-108 through 4-114. Such form serves as the applicant's attestation that he or she is a U.S. citizen or a qualified alien under the federal Immigration and Nationality Act, 8 U.S.C. 1101 et seq. (as such federal statute existed on January 1, 2009, or as it may be subsequently amended). If the applicant attests they are a qualified alien, Contractor shall verify the applicant's lawful presence in the United States using the Systematic Alien Verification for Entitlements (SAVE) Program.
