  • NPZA Scholarship applicants. To apply for a scholarship please complete and return two (2) forms. The first form is the NE APA/ NPZA scholarship application form. ( .DOC). The second form is the United States Citizenship Attestation Form, available on the State of Nebraska Department of Administrative Services website at www.das.state.ne.us.

    • This form is required because NE APA is a recipient of federal grant funds and as such the individual scholarship recipient receives public benefits and is contracted to comply with Neb. Rev. Stat. §§4-108 through 4-114. Such form serves as the applicant's attestation that he or she is a U.S. citizen or a qualified alien under the federal Immigration and Nationality Act, 8 U.S.C. 1101 et seq. (as such federal statute existed on January 1, 2009, or as it may be subsequently amended). If the applicant attests they are a qualified alien, Contractor shall verify the applicant's lawful presence in the United States using the Systematic Alien Verification for Entitlements (SAVE) Program.
